Salicylic acid (BHA): the exfoliant that cleans pores from within
Complete salicylic acid guide for acne and blackheads. COSRX BHA vs Paula's Choice, concentrations, betaine salicylate and how to combine it in your routine.
Salicylic acid is the only chemical exfoliant that can clean the pore from within. AHAs (glycolic acid, lactic acid) are water-soluble: they only exfoliate the skin's surface. BHA (beta-hydroxy acid) is oil-soluble: it penetrates through sebum and reaches the interior of the follicle, where it dissolves the plugs of keratin and oil that form blackheads and whiteheads.
This unique property makes it the most prescribed active for comedones (blackheads and whiteheads) and for oily skin with visible pores.
How BHA works
Lipophilicity: the key advantage
The stratum corneum (the outermost layer of skin) and the interior of pores are coated in lipids (sebum). Water-soluble actives cannot penetrate this lipid barrier. Salicylic acid can, because it is oil-soluble. It dissolves in sebum and accesses the interior of the follicle.
Once inside the pore:
- Dissolves keratin plugs (comedones) through keratolysis
- Loosens dead cells adhered to the interior of the follicle
- Reduces inflammation (salicylic acid has anti-inflammatory activity)
- Has mild antibacterial effect (reduces the population of C. acnes indirectly by cleaning the pore)
BHA vs AHA: when to use each
| Aspect | BHA (salicylic acid) | AHA (glycolic, lactic) |
|---|---|---|
| Solubility | Oil-soluble (penetrates the pore) | Water-soluble (surface only) |
| Best for | Blackheads, oily skin, pores | Texture, fine lines, dark spots |
| Depth | Inside the follicle | Superficial stratum corneum |
| Inflammation | Anti-inflammatory | Can irritate sensitive skin |
| Oily skin | Ideal (dissolves in sebum) | Less effective (sebum blocks it) |
| Dry skin | Can dry out further | Better option (hydrating) |
Arif, 2015 (Clinical, Cosmetic and Investigational Dermatology): Comprehensive review of salicylic acid as a peeling agent. Documents its comedolytic, anti-inflammatory efficacy and its capacity for intrafollicular penetration that distinguishes it from AHAs.
Concentrations and forms
Pure salicylic acid vs betaine salicylate
In K-Beauty, the most common form of BHA is not pure salicylic acid, but betaine salicylate (a gentler derivative of salicylic acid). The difference is important:
| Form | Potency | Optimal pH | Reference product |
|---|---|---|---|
| Salicylic acid 2% | Strong | 3.0-3.5 | Paula's Choice 2% BHA |
| Betaine salicylate 4% | Gentle (~equivalent to 0.5-1% SA) | 4.0 | COSRX BHA Blackhead Power |
| Salicylic acid 0.5% | Gentle | 3.5-4.0 | CeraVe SA Cleanser |
Lee et al., 2020 (Journal of Cosmetic Dermatology): Prospective multicenter study with salicylic acid-based products for mild comedonal-papular acne. Statistically significant reduction in inflammatory and non-inflammatory lesions at 4 weeks.
What concentration you need
- First time / sensitive skin: COSRX BHA (betaine salicylate 4%, very gentle)
- Persistent blackheads: Paula's Choice 2% BHA (real salicylic acid)
- Daily maintenance: CeraVe SA Cleanser (0.5%, cleanser)
- Body acne: Wash with SA 2% (back, chest)
The best BHA products

COSRX BHA Blackhead Power Liquid
4% betaine salicylate at pH 4.0. The best-selling BHA in K-Beauty. Gentle yet effective for comedones and texture. Ideal for skin that cannot tolerate pure salicylic acid.

Paula's Choice Skin Perfecting 2% BHA Liquid Exfoliant
2% real salicylic acid at pH 3.2. The most potent BHA in the cosmetic market. For persistent blackheads and resistant oily skin that needs more power than COSRX.

COSRX Acne Pimple Master Patch
Hydrocolloid patches for active pimples. They absorb pus, protect the lesion, and prevent manipulation. The perfect BHA complement: exfoliate at night, patch during the day.
How to use BHA in your routine
Protocol for blackheads
Night (every other night):
- Cleansing (double cleanse if you use SPF)
- BHA (COSRX or Paula's Choice). apply with cotton pad or fingers
- Wait 15-20 minutes (the pH needs time to work)
- Niacinamide (calms + sebum control)
- Moisturizer
Morning:
- Gentle cleanser
- Niacinamide
- Moisturizer
- SPF 50
Frequency
- COSRX BHA (betaine): can be used daily due to its gentleness
- Paula's Choice 2% SA: start 2-3 nights/week, increase gradually
- Never use BHA morning AND night: over-exfoliation
The 20-minute trick
BHA needs an acidic pH to exfoliate. If you apply moisturizer immediately after, you raise the pH and reduce efficacy. Waiting 15-20 minutes before the next step allows the BHA to work at the correct pH.
Combinations
BHA + niacinamide (the anti-oil duo)
The most effective combination for oily skin: BHA cleans the pores, niacinamide reduces sebum production. They complement each other without competing. Apply BHA first (low pH), wait, then niacinamide.
BHA + retinol/adapalene
Not on the same night. Both exfoliate through different pathways. Combining them the same night = over-exfoliation + destroyed barrier. Alternate: BHA Monday/Wednesday, adapalene Tuesday/Thursday.
BHA + AHA
Not together. BHA for inside the pore, AHA for the surface. Alternate nights: BHA for blackheads, AHA for texture and radiance.
BHA + BPO
Compatible. BHA exfoliates, BPO kills bacteria. Different mechanisms, they potentiate each other. Can be used the same night (BHA first, BPO after) or separated morning/night.
BHA for body acne
Salicylic acid is especially effective for acne on the back, chest, and shoulders (bacne). Body pores are larger and BHA penetrates more easily.
Protocol:
- PanOxyl wash 10% in the shower (2-3 min contact)
- Paula's Choice BHA Body Lotion post-shower
- Do not scrub with abrasive sponges (worsens inflammation)
Contraindications
- Aspirin allergy: salicylic acid is chemically similar to aspirin. Consult your doctor
- Pregnancy: topical BHA at cosmetic concentration (0.5-2%) is considered probably safe with limited use, but many dermatologists recommend avoiding it. Alternative: azelaic acid
- Active eczema or destroyed barrier: wait until it recovers
BHA: COSRX vs Paula's Choice
| Precio |
“BHA is the first-line active for blackheads and pores. COSRX BHA ($18) is the perfect entry point: gentle, effective, and lasts 3-4 months. If it is not enough, Paula's Choice 2% ($35) is the upgrade. Always combine with niacinamide to balance sebum control. And wait 20 minutes before applying the next step.”
